Modern healthcare is fragmented by design. You see one physician for a physical, another at urgent care, a third at the hospital, and none of them are holding the whole picture.     A long first appointment: full medical and family history, physical examination, and comprehensive baseline bloodwork.

  2. Planning

    We go through every result together, agree on what matters most to you, and build a written plan around it.

  3. Implementation

    The plan goes into practice with regular contact — adjustments, questions answered, and problems handled as they arise.

  4. Review

    Repeat testing at intervals shows what is working. The plan changes as your results and circumstances change.

Common questions

What is concierge primary care?

A membership model. Instead of billing volume, the practice charges a periodic fee and keeps the patient panel small. That funds longer appointments, direct access to your physician, and time spent on prevention rather than only on acute problems.

How is this different from a traditional practice?

Chiefly time and access. A conventional panel can run to two thousand patients or more, which forces short visits and long waits. A smaller panel makes same-day availability realistic and lets your physician actually track your numbers between visits.

Do I still need health insurance?

Yes. Membership pays for the primary care relationship, not for hospital admissions, surgery, specialist visits, imaging or medications. Keep your coverage in place for all of that.

Which office would I be seen at?

Dr. Eyerman sees patients at three locations in Bergen County — Fairview, Fair Lawn and Westwood. Most patients settle on whichever is closest to home or work, and the schedule is arranged around that. Call (201) 630-0810 and the office will sort out which location and day suit you.

What happens at the first appointment?

A full history, a physical examination, and baseline laboratory work. It runs considerably longer than a standard physical, because everything afterward depends on getting this part right.

How often will I see my physician?

As often as your health requires, plus scheduled reviews through the year. The scheduled visits are the ones that catch drift in your labs before you feel anything.

What if I need a specialist or end up in the hospital?

Referrals are arranged and the specialist is briefed directly. If you are admitted, your physician stays involved, follows the case, and helps you understand the decisions being made.
